Feeling optimistic about the left shift

21 October, 2026
< Back to news
Moving more care closer to home has long been an attractive idea. The real challenge is making it work in the day-to-day lives of patients, staff and local services.

There are reasons to feel cautiously optimistic. Local partnerships are developing new ways to bring general practice, community health, social care, local authorities and hospitals around shared needs. The strongest models rely on relationships, shared information, clear goals and outcomes that matter to people and communities.

The government’s 10 Year Health Plan for England sets out a shift from hospital to community, from sickness to prevention, and from analogue to digital. That gives the direction greater clarity. But policy alone cannot change where care happens or how resources flow.

There is still important work to do. Staff need time, skills and support to redesign services. Funding and incentives must enable new models, including during the transition, when old and new arrangements may need to operate side by side. Partners need to agree priorities, track results and adapt when an approach is not working.

Optimism is not a prediction of easy wins. It comes from a clearer understanding of what makes progress possible, and from local examples that others can learn from. The next step is to build on that experience patiently: start with a defined population need, involve the people delivering and receiving care, and use evidence to improve the model.

The promise of the left shift is care that is more timely, joined-up and shaped around people’s lives, wherever they live.
